The qualification path, and where the policies fit

Per ODP Announcement 25-013, the current process for new ID/A provider applicants. Steps 2 through 4 are where applications live or die, and every one of them runs on your written documents.

  1. Provider Applicant Orientation (PAO): your CEO personally completes the pre-registration webcasts and the full-day session, and passes the post-tests, to earn the Certificate of Achievement. ODP assigns your Administrative Entity (AE) based on your business address.

  2. The 60-day window: within 60 days of the date on your orientation certificate, submit your packet to the AE electronically: the ODP Provider Qualification Form (DP 1059), the New Provider Self-Assessment with your policies, procedures, and supporting documentation, and the Provider Qualification Documentation Record (insurance certificates, clearances, staff roster). Miss the 60 days and you are marked Not Qualified.

  3. AE review: the AE confirms receipt within 7 calendar days and reviews within 30. Deficient documents come back as an Additional Information Needed letter with 14 calendar days to resubmit.

  4. Decision by day 120: your orientation certificate expires 120 calendar days after issue, and the AE determines approved or not approved by that date. A Not Qualified determination means waiting 365 days from your PAO session before retaking orientation.

  5. Enrollment: once qualified, sign the ODP Waiver Provider Agreement, then submit your PROMISe enrollment application within 60 days of receiving the approved DP 1059, or the DP 1059 is invalidated.

  6. Access and start of services: after enrollment approval, ODP arranges your HCSIS/EIM access through your Master Provider Index (MPI) number. You may deliver a service only after an AE authorizes you in an individual's Individual Support Plan (ISP).

  7. Requalification: new providers requalify in the fiscal year immediately following initial qualification, then every 3 years on a cycle keyed to your MPI number.

What catches new ID/A providers off guard

The 60-day window is the whole game

Your policies, the New Provider Self-Assessment, and the Provider Qualification Documentation Record must reach your AE within 60 days of your orientation certificate date. Miss it and the application is marked Not Qualified (ODPANN 25-013).

Not Qualified costs a year

A Not Qualified determination means waiting 365 days from your most recent full-day orientation session before retaking PAO. The rework cycle inside the window (Additional Information Needed letters, 14 days to resubmit) is where weak documents die.

Your DOH home care license does not cover this

The Chapter 611 Home Care Agency license is a Department of Health framework. Chapter 6100 is Department of Human Services / ODP. Different code title, different agency, different policy set: your 611 manual satisfies none of the 6100 requirements above.

Training is counted in hours

DSPs and their direct supervisors need 24 documented training hours per year; management needs 12 (§ 6100.143). Orientation must happen before staff work alone with individuals (§ 6100.142).

EIM has named roles

Every provider designates an Incident Management Representative with end-to-end responsibility, who must become a Department-certified investigator within 12 months of taking the role (ODP Bulletin 00-21-02).

The rules move by announcement

Chapter 6100 has not been amended since taking effect in 2020, but the qualification process was rewritten by announcement in 2025 (ODPANN 25-013 replaced 24-002). Tracking announcements and bulletins is how a manual stays current.
