How these numbers are built
The source is the NPPES Data Dissemination file, the full public registry of every National Provider Identifier, published monthly by CMS. We count Type-2 (organization) NPIs whose primary taxonomy is 251E00000X (Home Health Agency) or 253Z00000X (In Home Supportive Care), grouped by practice-location state and enumeration month. Deactivated NPIs are excluded.
Primary taxonomy only, deliberately. An organization that lists home health as a secondary service alongside a hospital or nursing facility is a different business from one that leads with it, and counting every mention would inflate the totals with multi-service systems.
Three limits worth knowing before you quote this
- An NPI is not a licence. It is a federal billing identifier. State licensure is separate, varies enormously by state, and is not what this measures. These counts track market entry well and licensure only loosely.
- Older months are undercounted. Because deactivated NPIs are dropped, a historical month reads as “agencies registered then that are still active today.” Agencies that opened in 2019 and closed since are not in the 2019 figure. Recent months are close to complete; older ones drift low, so year-over-year comparisons across the trailing 24 months are sound while decade-long trends are not.
- State is practice location, not incorporation. A multi-state operator appears in the state of the address on the NPI record, so a chain with one registered entity serving several states counts once.
The aggregation is a single script, scripts/nppes-aggregate.py, run against npidata_pfile_20050523-20260712.csv. Anyone can reproduce or extend it.
How many home care agencies are there in the United States?
There are 122,620 active home care agencies in the United States as of Jun 2026. That figure counts organizations holding an active NPI whose primary taxonomy is Home Health Agency (251E00000X) or In Home Supportive Care (253Z00000X) in the CMS NPPES registry. Of those, 83,301 are registered as home health agencies and 39,319 as in-home supportive care providers.
How many new home care agencies open each year?
10,534 new home care agencies registered in the 12 months from Jul 2025 to Jun 2026, against 10,377 in the preceding 12 months, a change of +1.5%. That works out to roughly 203 new agencies a week nationally. Registration is a reasonable proxy for market entry because an agency generally needs an NPI before it can bill, but it is not the same as being licensed by a state.
Which state has the most new home care agencies?
Florida led with 973 new agency registrations in the 12 months to Jun 2026, followed by Ohio (913) and Texas (744). Ranking by new registrations rather than by total agencies surfaces where the market is actually moving, which is not the same as where it is largest.

Is an NPI the same as a home care licence?
No, and this is the main thing to understand before quoting these numbers. An NPI is a federal billing identifier issued by CMS. A licence is issued by a state agency under state law, and requirements vary enormously: some states licence home care agencies tightly, others barely at all. An agency can hold an NPI without a state licence, and in a few states the reverse happens too. These counts measure registration with the federal registry, which tracks market entry well and licensure only loosely.
